Remote Patient Monitoring (RPM) is a service that Health Wealth Safe provides your Doctor so that they can monitor your health in-between visits. 

1. We send you a device so that you can track your health. 

2. Then, our Care Managers will call you to check on you if you have any abnormal responses from you.

3. Your Doctor will then provide you care virtually, or in-person, depending on what you prefer.

Chronic Care Management (CCM) is a service that Health Wealth Safe provides to you and partnered care providers to keep you out of the hospital with preventative care planning and monitoring.

1. Care Managers will call you once a month to check in you to help you follow your care plan.

A Care Manager watches over health data and calls patients to ensure that they are following their care plans that the doctor ordered, or monitoring their health vitals. If there are abnormal health readings or if something seems off, Care Managers call patients to see what is going on, and to see if they would like to make an appointment with their doctor. All health data is directly connected to your doctor within seconds of using your health devices.
